Leab Awards 2009: Entry Form for Electronic Exhibitions

DEADLINE EXTENDED TO 31 OCTOBER 2008

Electronic exhibitions that were originally mounted on the World Wide Web between 1 September 2007 and 31 August 2008 by a U.S. or Canadian library or archival institution (including independent and public libraries and archives and those associated with colleges, universities, historical societies, and museums) are eligible. Electronic exhibitions should have a stable URL address and should be available online through September 2009 to allow adequate time for public viewing. Announcement of the winning exhibitions will be made in the spring of 2009. Institutions that sponsor winning exhibitions will receive award certificates at the 2009 American Library Association Annual Conference in Chicago.

To submit an electronic exhibition for the Leab Awards competition, follow the instructions below. While it is sufficient to submit urls of the electronic exhibitions for judging, institutions are also encouraged to submit the exhibitions in CD-ROM or other stable storage format (3 copies) for archiving at the depositories: the Grolier Club (New York City), the Harry Ransom Center, University of Texas, Austin, and the Bancroft Library, University of California, Berkeley. There is, however, no commitment on the part of RBMS / EAC or the depositories to migrate these formats.

The deadline (postmark or shipping date) for submitting entries for the 2009 awards is 15 31 October 2008.

Instructions for downloading, filling out, and submitting the entry form:

  1. Please read all instructions below before downloading and completing the form.
  2. The entry form is an Adobe PDF format document and is designed to be viewed and filled out using Adobe Acrobat / Adobe Acrobat Reader software. If you do not have either Adobe package already installed on your workstation, you can download and install Acrobat Reader for free from the Adobe Web site: (http://www.adobe.com/products/acrobat/readstep.html).
  3. If you have access to the full, licensed version of Adobe Acrobat, please use it to process your form, as it will allow you to save and submit an electronic copy of the completed form, which we greatly prefer. If, however, you are only able to use the basic Acrobat Reader, please follow the corresponding instructions below.

If your are using the full, licensed version Adobe Acrobat:

  1. Download the form by clicking on the link below, then open it using Adobe Acrobat (make sure you are using the full version of Acrobat, and not the basic Reader or browser plugin). Save a copy to your local hard drive, preferably with a different name, such as electronic_entry_myexhibitname.pdf.
  2. Fill in all of the required fields by typing in the appropriate boxes or blanks (you can use your tab key to move from field to field).
  3. Save the form again when once you have finished completing the form.
  4. Attach the completed, saved copy of your form to an e-mail message addressed to Richard Noble, RBMS Exhibition Awards Committee Chair (2007-2009) at Richard_Noble@brown.edu.

If your are using the basic Adobe Acrobat Reader / PDF Viewer Web browser plugin:

  1. Download the form by clicking on the link below. Depending on how your Web browser preferences are configured, this will cause your browser to load the PDF Viewer plugin and display the form in your browser window, or will cause the Acrobat Reader application to launch independently and display the form.
  2. Fill in all of the required fields by typing in the appropriate boxes or blanks (you can use your tab key to move from field to field).
  3. Important: Because the basic Reader / PDF Viewer plugin software will not save the information you have typed into the form, you must print the form and mail or fax copies to complete your submission. If you try to save a copy of the completed form to your hard drive, the saved file will not contain any of the information you have typed in, just the form template!
