Leab Awards 2010: Entry Form for Electronic Exhibitions
Electronic exhibitions that were originally mounted on the World Wide Web between 1 September 2008 and 31 August 2009 by a U.S. or Canadian library or archival institution (including independent and public libraries and archives and those associated with colleges, universities, historical societies, and museums) are eligible. Electronic exhibitions should have a stable URL address and should be available online through September 2010 to allow adequate time for public viewing. Announcement of the winning exhibitions will be made in the spring of 2010. Institutions that sponsor winning exhibitions will receive award certificates at the 2010 American Library Association Annual Conference in Washington, DC.
To submit an electronic exhibition for the Leab Awards competition, follow the instructions below. While it is sufficient to submit urls of the electronic exhibitions for judging, institutions are also encouraged to submit the exhibitions in DVD, CD-ROM or other stable storage format (4 copies). Physical media become part of depository collections, available for public consultation, at the Grolier Club (New York City), the Harry Ransom Center, University of Texas, Austin, and the Bancroft Library, University of California, Berkeley. There is, however, no commitment on the part of RBMS / EAC or the depositories to migrate these formats.
The deadline (postmark or shipping date) for submitting entries for the 2010 awards is 15 October 2009.

- Please read all instructions below before downloading and completing the form.
- The entry form is an Adobe PDF format document and is designed to be viewed and filled out using Adobe Acrobat / Adobe Acrobat Reader software. If you do not have either Adobe package already installed on your workstation, you can download and install Acrobat Reader for free from the Adobe Web site: (http://www.adobe.com/products/acrobat/readstep.html).
- Download the form by clicking on the link below. Depending on how your Web browser preferences are configured, this will cause your browser to load the PDF Viewer plugin and display the form in a new browser window, or will cause the Acrobat Reader or full version of the Adobe Acrobat application to launch independently and display the form.
- Fill in all of the required fields by typing in the appropriate boxes or blanks (you can use your tab key to move from field to field). The size of the font will scale to fit the amount of text that you enter. You can save your work as a PDF formatted file to complete later or as a record.
- When you are satisfied with the contents of the form, click the "Submit and print" button. The form will be copied into an email to Richard Noble (Richard_Noble@brown.edu), RBMS Exhibition Awards Committee Chair, using the settings in Acrobat Reader. Send this email using your normal process. After you send the form, a print dialog will open.
- If you are able to supply DVDs, etc., please send 4 copies, together with 4 copies of the entry form, to Richard Noble, RBMS Exhibition Awards Committee Chair (2007-2010) / John Hay Library / Brown University / 20 Prospect St. / Providence, RI, 02912 / 401-863-1187. Mr. Noble will acknowledge receipt of submissions by email.
- All information in the form, with the exception of the contact name and address, may be published in printed and online lists of entries. The forms themselves are circulated to committee members and are eventually retained in the Leab archives.
